      Please provide us with contact information for your Youtheatre field trip.

    • Section 2 - Production Information

      Below, please provide us with information regarding the Youtheatre productions you would like to attend.
    • Please list the number of students that will be attending. Tickets are $9/ticket for all shows except Peking Acrobats, which is $12/ticket.
    • You will receive 1 free adult ticket for every 15 student tickets. Any additional tickets will be charged at the regular ticket price.
    • Please provide us with the grade(s) of the attending group. Use CTRL key to select all that apply.
    • Please list the number of buses you will be arriving in. Providing this number is important to the arrival process. If you will be arriving in cars, please write "Cars" in the blank field.
    • Please indicate yes below if you would like to apply. Upon completion of this form you will be taken to a confirmation page. At that time you will find a link to apply for a grant. Title 1 Illinois School Report Card will be required.
    • Please list the number of students/ teachers who will be attending in a wheel chair. Please list the number of students that will need use of a theater issued hearing devise. Please give us any additional information that can best help us prepare for your group.

      Thank you for filling out this Youtheatre group ticket request form. Please expect an email including a contract / invoice with all payment information. A deposit of 20% of the fee will be due within two weeks of receiving the contract. Final payment is due 30 days prior to the production. If payment is not received, your reservation may be cancelled. If you have any questions, please contact our Youtheatre Department at Youtheatre@NorthShoreCenter(dot)org.
